Answer the following questions. Complete this form and return to your child’s teacher no later than Monday, August 12. The school has designated Friday, August 23 as INTAKE Conference Day, and most of the conferences will take place on this date along with additional dates that the classroom teacher(s) will provide. INTAKE Conference times will be published as school begins and you will receive instructions on how to access the online conference sign-up system.
